HART (Home Assistance Referral Team), Inc. is a non-profit organization whose mission is to enable members of our community to maintain quality of life in their own homes by linking them with qualified and affordable help. We connect those needing in-home care with high-quality caregivers in both Niagara and Erie Counties.

Our Caregivers can provide the following services:

- Skilled Nursing Care
- Health and Personal Care
- Respite for Family Caregivers
- Meal Planning and Preparation
- Companionship
- Shopping and errands
- Drive to appointments
- Light housekeeping and yard work

We can help you find assistance whether your needs are for three hours or 24 hours per day, once per month or 7 days per week. We have Caregivers who provide a variety of services to meet your specific needs. Our staff will review your needs and schedule, and match them with our Caregivers credentials and schedule. Each Caregiver has been personally interviewed by HART staff, had professional references checked and a background check performed.

HART is a private pay organization. Some insurances, especially long-term care insurance, will accept a HART Caregiver. Check with your insurance provider prior to calling HART.

Call (716) 754-8313 Monday through Friday 8:00 am-4:00 pm to speak with one of our staff or leave a message and a staff member will return your call the next business day. Before calling, please have the following information available:

- Schedule – # of hours or specific times, days of the week, times of day.
- Address and phone number of those needing assistance.
- Health issues
- Home environment (animals or smokers in the house)

We recommend a personal interview as well as a phone interview when hiring one of our Caregivers.
