Niagara Falls Memorial Medical Center, a state designated Vital Access Provider, sponsors and administers the NFMMC Adult Health Home. As such, the Health Home is enrolled in the NYS Medicaid program.

NFMMC is dedicated and committed to providing services to the neediest and most vulnerable populations in Niagara County. We believe connecting with the community is a vital part of who we are. NFMMC has six primary care sites located in the county and offers transportation to these services when needed. The NFMMC HH has built a network now comprised of 200+ Health Home partners throughout Niagara County.
