BFNC prepares, equips and empowers individuals to address life management issues, and to improve the quality of life in their communities.

The Buffalo Federation of Neighborhood Centers plays a major role in ensuring that anyone in Western New York who needs help and has no place to turn, or who has had trouble accessing services, or who has slipped through the cracks of the human service system, has a trusted place to go for help in taking the next step toward maximum independence.

Services & Programs:
- Care Management: A BFNC Care Manager will work with you to understand what you need to stay healthy. You may need housing. You may need help getting enough food. You may need transportation. You may have problems getting your medications or taking them. You may use the emergency room a lot or end up in the hospital often. Once the care manager knows what you need, she/he will work with you and the BFNC Care Management team to help you get those services and to teach you how to stay healthy.

Receive 24-hour on-site staffing, meals, support, advocacy and recovery-oriented services. Involvement in activities outside the residential setting include recovery oriented programs, vocational programs, school, and competitive- and non-competitive employment.
